IN OFFICE and/or TELEHEALTH ASSESSMENTS:
- New Client- Initial Evaluation and medication management $350 (45 minutes -1 hour) In person and/or TELEHEALTH
- Established Client- Follow-Up Appointment $175 (15-25 minutes) In person and/or TELEHEALTH
IN HOME ASSESSMENTS:
New Client- Initial Evaluation and medication management $600 (1 -1.5 hour) IN HOME Visit with travel fee based on location.
Established Client- Follow-Up Appointment $400 (30-45 minutes) IN HOME Visit with travel fee based on location.
MEDICATION ADMINSTRATION:
Medication Injection Visit $125 (10 minutes) In Office
Medication Injection Visit $250 (10 minutes) In Home with travel fee based on location.
EMERGENCY VISIT ASSESSMENTS:
In office and/or Telehealth -New Client Evaluation and medication management Starting at $450 based on complexity.
In office and/or Telehealth -Follow Up Client Evaluation and medication management Starting at $300 based on complexity.
In Home Visit-New Client Evaluation and medication management Starting at $650 based on complexity with travel fee based on location. FULL FEE Required.
In Home -Follow Up Client Evaluation and medication management Starting at $400 based on complexity with travel fee based on location. FULL FEE Required.
PAPER WORK FEE:
Forms for Disability, FMLA and other paperwork have a processing fee of $25.00 to $75.00 per page of the paperwork, depending on the complexity and time required to complete the paperwork. These services are NOT covered by insurance. This fee must be paid by you prior to completion of the paperwork.(all paperwork fee require an initial client visit):
Completion in letter format Emotional Support Animals, Academic or Work Accommodations, FMLA, Short Term Disability, or Return to Work) $75
Form Completion for Emotional Support Animals, Academic, Work Accommodations, and Return to Work) $50-75 per page.
Form Completion for FMLA, Short /Long Term Disability $25-75 per page.
Pharmacogenomic Testing
Testing is covered by most insurance companies.
